Dementia Care Officer

Contact Bolton Dementia Support today to see how they can help you and those you care for.

Our Dementia Care Officer (DCO) will provide people living with dementia (PLWD), their families and carers with practical support, advice and access to resources, activities and community groups that will sustain, as far as is possible, people’s independence, existing lifestyle and relationships.

The DCO will be your first point of contact when referred and be your named contact throughout. They will assess, triage and then signpost to information, local groups and services that will promote physical, mental, and emotional wellbeing for both the PLWD and their carer/s and enable them to lead their own lives. The DCO aims to promote our motto of ‘Living well with dementia’.

The DCO will work to ensure people with dementia are not excluded from services because of their diagnosis, age, communication difficulty or any learning disability.

A more involved service covering for a family crisis.

The Advocacy service can accompany a carer or PLWD to a multi-disciplinary meeting and advocate on their behalf. It can be daunting to attend meetings regarding your loved one if they are involved with a lot of agencies. Our DCO can accompany you to these meeting and even speak on your behalf if you feel unable to do so. The DCO will also offer support and assistance to carers and the person they are supporting. We tailor the service to your needs.

At the end of the term of involvement the DCO will work with the family to formulate an effective exit strategy which is acceptable to all parties and help to involve other agencies if necessary.
